Support Our Clients

We support our clients to attend local outpatient appointments and liaise regularly with care co-ordinators and other professional services involved in their care to ensure that our clients are receiving the best quality of on-going support.

Centred Support

City Support provides a person-centred support that is based around each client’s individual needs. We acknowledge that the same diagnosis may have a different impact on each individual client and we adapt their support plan accordingly.

Responsive

We are responsive to the cultural, gender and any specific needs of the client in developing their support plan. Whatever the need, support is provided with patience and understanding, whilst at the same time, provided within appropriate boundaries..

Exclusive Plan

Each Client’s support plan is exclusive, with the aim to promote good health, increase independence and promote good quality of life. We actively encourage Clients and appropriate significant family members and support network to participate in the support planning process.

Health Service Support

We support client to register with the local primary health care service. However, if they wish to remain with their current GP and the GP is willing to retain them, clients may choose this option

Medication Support

We support clients with their medications, we order repeat prescriptions and collect from the preferred pharmacy, we also support client in taking their medications at the right time in the right dosage to prevent any other health complications, overdose or non-compliances.

City Support ensures good communication practice amongst the team and liaise effectively with all professionals, and with support that is flexible to meet the diverse needs of each client.

Our support programmes are tailored to improve client’s health and life skills. We have trained and dedicated staff who can provide one-to-one support where required in all areas of daily living, including support with finances and other resources, tenancy management, community integration, a positive structure be it education or work.

We value promoting positive well-being and work with our clients to understand how good diet and exercise can promote positive mental health.
